Things to Bring

Legal Guardianship paperwork
-If you are not the biological parent (natural mother or father) of the child, please bring LEGAL documents of your right to give consent for the procedure and general anesthesia.

Insurance Cards

Child's favorite stuffed animal or blanket to stay with the patient during the procedure.

Check In

You will be checked in by our administrative assistant at our front desk upon arrival. You will also sign the consent for surgery and anesthesia at this time. After consents are signed you will be placed in our prep/recovery room to begin the process, where you will also remain until your child is discharged.

*****EATING AND DRINKING IN THE PREP/RECOVERY ROOM PRIOR TO SURGERY IS PROHIBITED SINCE OUR PATIENTS ARE FASTING.


*Please be patient with us. You may be asked the same questions by different people throughout your check-in process. This is part of the safety check before your child goes into surgery.
